With on-site allied health and pathology, we make it easier to keep your care joined up.
What Is a Chronic Condition?
A chronic condition is a long-term health condition that has lasted, or is likely to last, six months or longer. Many chronic conditions cannot be cured, but with the right care and support, most people can manage their condition well and continue to live a full and active life.

Our GPs support patients managing a wide range of ongoing conditions, including:
- Diabetes and pre-diabetes
- Asthma and COPD
- Heart disease, high blood pressure and high cholesterol

- Thyroid and other hormonal conditions

- Mental health conditions

Care Plans and Coordinated Care
If you have a chronic condition, your GP may prepare a chronic condition management plan (previously known as a GP Management Plan and Team Care Arrangement). This sets out your health needs, your goals and the steps to get there, and helps coordinate your care across your GP and other health professionals. If you are eligible, it can also give you access to a Medicare rebate for a number of allied health visits each year.
Allied Health Under One Roof
One of the benefits of managing your condition with us is that much of your care team is in the same place. If your plan includes allied health, you can often see our on-site physiotherapist, dietitian, podiatrist and psychologist, which makes appointments easier to attend and helps everyone stay on the same page.

Ongoing Reviews and Support
Managing a chronic condition is an ongoing process. Your GP will review your plan regularly, track your progress, adjust your care as needed, and make sure you have the support you need along the way. Regular reviews help catch any changes early and keep your care on track.
Bulk Billed
Chronic condition management plans and reviews are bulk billed for patients with a valid Medicare card. If your plan includes allied health, a Medicare rebate may apply for a number of visits each year for eligible patients. See our Booking and Fees page.
Availability
Appointments are available 7 days a week, 8:00am to 5:00pm. Longer appointments are available for care planning and reviews, just let reception know when you book.

What is a chronic condition management plan?

Am I eligible for a care plan?
